Beware: Barrett's Esophagus Increases Risk of Deadly Cancer

Everybody, the question that I was asked is what is Barrett's esophagus. That is a disease entity that's near and dear to my heart. As I do a ton of research and Barrett's esophagus just to simplify it. I usually tell my patients is when the lining of the esophagus dress, look like the lining of the intestine that happens in response to acid reflux, that comes upward from the stomach into the esophagus and then injures esophagus. And then the esophagus is smart organ. And It starts to look like the lining of the intestine to better handle the acid. So why do we care about it? We care because Barrett's is the main risk factors or esophageal adenocarcinoma. Which is a very fast rising cancer in the United States right now. So, who should we screen? Who should be worried about this? So according to the American College of gastroenterology at screening endoscopy should be indicated for patients and I'm just going to read it off their website for patients with chronic girl. And three or more additional risk factors for Barrett's esophagus, which include the male gender, 50 years. Over 50 years old white race tobacco, smoking obesity, and a family history of Barrett's esophagus or esophageal adenocarcinoma and first-degree relative. So again with those risk factors, it's important that patients receive a screening endoscopy, so care should be sought to a gastroenterologist if if any of you meet any of that. Criteria. This is really important because we're increasingly recognizing that the vast majority of people that meet these qualifications are not being screened. There are numerous studies showing in large populations. Only 30 percent of people who are eligible for screening or actually screen. Is it safe to take proton pump inhibitors, such as Prilosec, long term?

Long-term use of Prilosec & other PPIs: Are they safe?

Hi everybody, I hope you're doing well. The question I was asked to answer, is it safe to take proton, pump inhibitors? Such as Prilosec long-term ppis bind to the acid pumps in the stomach, and inhibit secretion of acid. They work well, in disease states such as gastrointestinal reflux disease and ulcers in the stomach and the intestine. However, their long-term use has been associated with several safety issues. I pulled up this cartoon from a review article and it could be seen here that they've been implicated in brain issues such as dementia. To the lung, to the heart, to the kidney comes in, chronic renal issues and to the bones are some of the common issues. However, there are many issues with the quality of these studies that go beyond the scope of this video. Recently, those are very well done. Study published in gastroenterology our major journal back in 2019 that followed over 17,000 patients, where some were given a PPI and and some were not and they followed these patients for over three years and they found that ppis or were not associated with any major Adverse Events from that cartoon that I showed you. With the exception of mild infections that could be easily treated. So in short long term, use of ppis for the right indication are considered safe in the gastroenterology Community anytime, anybody is using ppis for long-term use, it's appropriate to be under the care of a primary care physician or gastroenterologist. GERD Treatments: Lifestyle Changes, Medications & More

Everybody. Hope you're doing well. The question is what treatments are available for GERD? Card stands for gastrointestinal reflux disease. Kurt is defined as a passage of contents from the stomach going into the esophagus. Generally patients feel pain in that direction is usually as described as a sharp pain that travels upwards. The first thing to realize is to make sure that there is not related to the heart. So just generally left-sided pain or pain on exertion. And it's also important to make sure there's no alarm symptoms. If there are alarm symptoms patients, should seek urgent care. So what do we mean by that pain? With eating food food? Getting stuck in the esophagus weight loss, vomiting, or even signs of bleeding, which is vomiting blood or black stools. Initial management of GERD. Generally consists of lifestyle modifications. So weight loss with has been weight gain or if somebody's overweight elevation of the Head of the bed at night and then avoid dietary triggers spicy foods, caffeine mint, chocolate carbonated beverages peppermint. Those are all foods that have classically been known to cause reflux and then there's two classes of medications one which is known as a histamine blockers. The most common is from auditing so over the counter and then the other one is proton pump inhibitors. That actually blind to the acid pumps in the stomach. Like those generally work. Key Foods to Avoid for Acid Reflux: Fats, Citrus, Chocolate

I hope you're all having a good day. The question I was asked was what foods should be avoided if you have acid reflux. So I generally there are some foods that could lower the pressure of the lower esophagus and therefore cause acid which is made in the stomach to come up words into the esophagus and that's where you feel that classic burning sensation in your mid sternum in your chest area. The foods I classically do it are high fat foods. I don't need to go through that. I know you guys know what but that's a foods like your Fries at McDonald's or your Big Mac, things like that. So, avoid the fast food, tomatoes and citrus are classic ones. The tomato sauce on pizza, or common trigger Citrus such as oranges. Limes, pineapple are classic triggers and then other classic foods are chocolate. I know you didn't want me to say that, but chocolate sequester, the classic trigger. So, try to avoid the chocolate and then things like, caffeine mint, alcohol carbonated beverages can also Becoming trigger. So if you know you're going to be having a day full of these triggers, you may want to consider an antacid to help you get through the day, to avoid the gerd.
